SUPPLEMENTAL PERSONAL SPORT ACCIDENT INSURANCE:

If a regular or casual curler who is a member of an affiliated club has an accident while curling, please follow this process:  

  1. Gallagher must receive notification of your accident within 30 days of it occurring and
    receive your claim form within 90 days of the accident.
  2. Complete Sport Accident Claim Form and Physician Statement. If your claim is for
    dental injury have your dentist complete and submit a Predetermination Form.
  3. Claims Forms can be submitted to our office electronically or by fax. If you are submitting the forms by mail, please forward copies only and retain originals for your files.

  • Sanctioned Activity” means a curling activity, event, competition, league, bonspiel, practice, clinic, training session, or meeting that has been approved, organized, or recognized by Curling Alberta

Personal Accident Insurance Update

Wawanesa is our Personal Sport Accident Insurance Provider.

If a club member or casual curler has an accident while curling, please follow this process:  

  1. Club GM or Designate helps member complete the appropriate claim form (see form links below for: Member curler OR Casual/Drop-in curler) & submit to: [email protected]
  2. A Wawanesa Claims Specialist will review the claim & guide the club member or casual curler through the rest of the process.
